High loss with Netgear Nighthawk in IP pass-through mode

Hi,
I've got a Nighthawk M6 (MR6550 variant) that I'm using with T-Mobile for business 5G, and if I connect it to my WAN port in routing mode (which puts me into dual-NATting and stops IPsec from working) then I get good speeds (660/46mbs) and no packet loss at all, and low latency (about 80ms).
If I put the hotspot into IP pass-through mode (which is what I bought it for, since I have a static public IP address and don't trust a router I don't control) then I get 62% packet loss and the latency goes up to about 115ms but spiking to as high as 2820ms.
The Netgear forums say that other people have add similar issues with IP pass-through being unreliable, some even saying that Amazon Cloudfront CDN's work poorly or stop working altogether for them.
Anyone else experienced this or know what might be going on?
